This study day is an important opportunity for Practice based lead GPs and Practice Nurses to learn more about the Somerset Diabetes Service, update their knowledge, address problem areas and agree future actions.

Two separate additional workshops are being run with Somerset Community Education Provider Network (CEPN) and SGPET we are pleased to invite Health Care Assistants working in General Practice (topics to include understanding more about diabetes, why we need to work together to improve care, 5 ways on how you can make a difference day to day and preventing problems) and Health Care Assistants working in Care Homes (topics to include understanding more about diabetes and prediabetes, why we need to work together to improve care, 5 ways you can make a difference day to day, preventing problems, enabling self care) to attend session relevant to them.
